The effect of training and telephone follow-up based on the chronic care model on self-management, quality of life and patient satisfaction in patients with ischemic stroke

Abstract (EN)

Objective: The aim of this study was to evaluate the effect of education and telephone follow-up based on the Chronic Care Model on self-management, quality of life and patient satisfaction in patients with ischemic stroke. Method: The study is a randomized controlled experimental study. This study was conducted between September 2018 and September 2019 with a total of 68 patients, 34 intervention and 34 control groups who were treated with the diagnosis of ischemic stroke in the Neurology Clinic of Akdeniz University Hospital. Personal Information Form based on the Chronic Care Model, Stroke Self Efficacy Scale and Stroke Self-Management Assessment Form to assess the self management, Stroke-Specific Quality of Life Scale and Patient Assessment of Chronic Illness Care were used to collect data. Within the scope of the StrokeCARE intervention protocol, the patients in the intervention group were given one-to-one training before discharge, then the patients were supported with the training booklet, telephone monitoring and reminder SMS applications were made. The pre-test data were collected in the hospital while the patients were hospitalized, and the post-test data were collected at the 3rd month of discharge. Statistical analysis of the data was performed in SAS 9.4 package program. Results: It was determined that the mean scores of the self efficacy and the quality of life were higher in the intervention group compared to the control group, but this difference was not statistically significant (p>0.05), and the chronic disease care satisfaction scores were statistically significantly higher (p<0.05). It was determined that the level of stroke knowledge (p<0.05); cholesterol levels and healthy lifestyle behaviors in the intervention group improved (p>0.05). Conclusion: In the study, it was found that the StrokeCARE intervention protocol was applicable, and this protocol improved the self-management and quality of life and increased satisfaction levels of patients with ischemic stroke.
