Abstract (EN)
8.SUMMARY In everytype of trauma metabolic and endocrynologic changes happen according to seriousness of trauma.These changes are the efforts of the organism to protect hemostasis towards trauma.lt is well known that tissues injure after trauma and as a result of this,acute phase reactants release from the liver to blood.în these C-reaktive protein is a well known acute phase reactant.In traumatic brain injury a glycolityc enzyme,Neuron Spesific Enolase(NSE) that only exists in neuron can be found at increased levels in serume. Head trauma can be observed in %80 of all multitrauma patients and in %75 of traffic accidental deaths.Traumatic brain injury is,also,one of the most important causes of morbidity and mortality in developed countries.Only %40-50 serious head trauma patient improve functionally. In our study 100 patients who were admitted to Emergency department( ED) because of head trauma was examined.Patients according to age groups were seperated to two groups;group l:Pediatric group (0-16 years, 69 patients) and group 2:Adult group (over 16 years, 3 1 patients). According to serioussness of head injury patients were seperated to three groups. Group 1 : Mild head injury (GCS=13-15, 35 patients); group 2:Moderate head injury (GCS=9-12, 31 patients) and group 3:Coma or severe head injury (GCS=3-8, 34 patients). For every patient IL-1, IL-2, IL-6, IL-8, TNFa, Hs-CRP and NSE values were examined. Revised Trauma Score (RTS) and Glasgow Coma Score (GCS) were calculated for every patient. In our study we found that the more seriousness of the head injury occurs,the more significant levels of NSE, Hs-CRP and IL-6, IL-8, TNFa levels become. IL-1 and IL-2 levels couldnt significantly demostrate the seriousness of head injury. Meanwhile NSE, Hs-CRP and IL-6, IL-8, TNFa levels were significantly higher in non survivors than in survivors.In our study, we introduce physicans biochemical paremeters of sitokines like IL-6, IL-8 and TNFa, and trauma scores like GKS, RTS and NSE, Hs-CRP as the independent predictors of mortality of posttraumatic early period in blunt head injuries. 72
