Investigation of the prediction of the systemic immune-inflammation index in patients diagnosed cardiac syndrome X

Abstract (EN)

Cardiac syndrome x is a clinical picture in which typical anginal chest pains and a positive cardiac stress test are present without coronary artery disease. There are many unknown aspects regarding the etiology of cardiac syndrome x disease. Although it is estimated that many systems play a role in the pathophysiology of cardiac syndrome x, factors such as insulin resistance, abnormal cardiac pain perception, microvascular spasm, decrease in estrogen level and abnormal coronary flow reserve, there is no clear information about its pathophysiology. In today's studies in the literature and clinics, it is stated that the decreased coronary vasodilator response associated with increased coronary resistance in individuals with cardiac syndrome x disease develops on the basis of endothelial dysfunction and inflammation. For this reason, in this study, it was aimed to determine whether there is a relationship between systemic immune-inflammation index levels, which have been shown in many studies before, and patients diagnosed with cardiac syndrome x, which have a close relationship with important cardiovascular events such as coronary artery disease, heart failure and hypertension. The study group consists of 80 patients who underwent outpatient coronary angiography in the cardiology department of Fırat University Hospital between October 2021 and April 2022. The study group was diagnosed with cardiac syndrome x based on the presence of exercise-induced typical angina pectoris and ischemia in myocardial perfusion scintigraphy performed with angiographic intervention from normal coronary arteries. In the control group, 80 people without a diagnosis of stress test were included by matching their gender and age. It was determined that there was no statistically significant difference between the study and control groups according to age, gender, body mass index, hypertension, smoking, diabetes mellitus, dyslipidemia and family history. It was determined that there was a significant difference between the hs- C Reactive Protein levels of the individuals in the study and control groups (p=0.028), and the systemic immune-inflammation index levels measured in the samples taken from the patients were higher in the individuals in the control group (p=0.003). As a result of our observations, our study is the first study to examine the prediction of systemic immune-inflammation indices with cardiac syndrome x, which is mainly blamed for endothelial dysfunction and inflammation in its etiology. As a result of this study, a statistically significant difference was found between the control and study group in terms of systemic immune-inflammation index parameter. It is suggested that there is a need for extensive research on the pathophysiological role and clinical significance of systemic immune-inflammation index parameter levels in patients with cardiac syndrome x.
