The effects of rosuvastatin to reduce systemic inflammatory response in coronary artery bypass graft patients done with cardiopulmonary bypass technique

Abstract (EN)

The most prominent complication of cardiopulmonary bypass (CPB) technique, which is performed today for open-heart surgeries, is systemic inflammatory response, and its indicators are change in cytokine amounts, complement activation, and changes in acute phase reactants. In this study, the effects of rosuvastatin on the CPB applied patients? IL-6, IL-10, IL-18 and high sensitivity C-reactive protein (hs-CRP) values were researched.In the study, 25 of 50 patients that were applied coronary bypass surgery were evaluated as rosuvastatin group and 25 as control. For the treatment group, rosuvastatin 20 mg. tablet was started to be given once a day from 7 days before the operation until the 28th postoperative day. For the levels of IL-6, IL-10, IL-18, blood samples were taken before induction of anesthesia (T1), during CPB (T2), at the 5th minute after cross-clamp was taken, (T3), after protamine infusion (T4), on the 3rd postoperative day (T5) and on the 28th postoperative day (T6). For hs-CRP and cholesterol profile, the samples were taken before the induction of anesthesia (T1) and on the 28th postoperative day (T2).In biochemical analysis, by the use of rosuvastatin, statistically meaningful decreases were observed in IL-6 levels at T4 (p<0.01), T5 (p<0.001) and T6 (p<0.01) times and a statistically meaningful increase was observed in IL-10 levels at T3 (p<0.01) and T4 (p<0.05) times. In the samples taken for hs-CRP, statistically meaningful decrease was detected at T2 time by the use of rosuvastatin (p<0.001).Consequently, CPB technique linked systemic anti-inflammatory response was reduced at certain times, being more intensive in intraoperative periods, by using daily 20mg. of rosuvastatin from 7 days before the operation until the 28th postoperative day in the patients having coronary heart disease and those who were applied coronary bypass surgery due to these diseases.Keywords: Cardiopulmonary bypass, systemic inflammatory response, rosuvastatin.
