Abstract (EN)

Objective: The aim of this study is to provide prospective examination of patients presenting with cardiovascular drug intoxication. Materials and Methods: The study was conducted and applied to the Department of Emergency Medicine of Çukurova University Faculty of Medicine between 2017-2020. The patients admitted to our center for cardiovascular drug intoxication due to accidental intake or suicide attempts included. 25 consecutive patients over 18 years of age who admitted to our department with cardiovascular drug intoxication and who accepted to participate in the study were accepted to this study. No external laboratory parameters related to normal clinical process has been studied and no treatment different than the literature has been applied. In addition to the demographic characteristics of the patients, the type of drugs or substances tat they took, the ways of taking and the dosages were recorded in detail. During the emergency treatment and hospitalization, the time of development of vital signs such as shock or bradycardia, treatment of intoxication including; antidotes has been used, type of extracorporeal methods and invasive procedures were recorded in detail. SPSS v.23.0 program was used for statistical evaluation of the obtained data. The outcome of the patients included in the study was reported. Findings: Twenty-five patients admitted to the emergency department with cardiovascular intoxication were included in the study. Eight (32.0%) patients were male and 17 (68.0%) were female. Physical examination findings were; 4.0% (n: 1) of the patients were in poor condition. To treat this patients in the emergency room; Calcium to 52.0% (n: 13), Glucagon to 36.0% (n: 9), Lipid to 32.0% (n: 8), Lipid to 12.0% (n: 3) Atropine positive to 20.0% (n: 5) Inotropic, 4.0% (n: 1) intubation and 4.0% (n: 1) were used and Pacemakers, Hemodialysis, Hemofiltration, Plasmapheresis and Intraaortic balloon pump were not used. Result: According to the results to determine the type of intoxication type without delay and to apply the necessary antidote treatment and extracorporeal methods in toxication patients presenting to the emergency department by taking medications with cardiac effects may increase survival rates. It was found that 56.0% (n: 14) of the patients were discharged after the treatment, 44.0% (n: 11) of the patients voluntarily left the hospital and no deaths were found. It was seen that patients who took calcium channel blockers had more cardiogenic shock than those took beta blockers and bradycardia more than those receiving calcium channel blockers. It was observed that the patients who received calcium channel blocker had a high level of drug blood levels and clinical findings were not correlated with high levels of drug in patients receiving beta blockers.
