Kienbock's disease, the place of radial shortening inlichtman classification stage 2, stage 3A and stage3B

Özet (EN)

Purpose: In Kienböck's disease there is different treatment approach for every stage. In Lichtman stage 1 general approach is conservatively following but in stage 2 and stage 3A radial shortening is done. There are discussions about radial shortening in Lichtman stage 3B but in despite of discussions we conducted radial shortening on stage 2,3A and stage 3B to analysis result of radial shortening. Material and Method: 31 treated patients attended to study in Gaziantep University Şahinbey Research and Training Hospital between 2009 to 2019. Study was done retrospectively. Patient who had have surgical operation on his/her wrist and have another diseases other than avasculer necrosis of lunate bone was excluded from study. Nakamura, VAS score and Modified Mayo wrist score were analyzed. MRI and X-Ray, X-Ray were used radiological evaluation in preoperatively and postoperatively, respectively. Findings: The ratio of patients was male/female 18/13. 11 of them were stage 2, 8 were stage 3A and 12 of them were stage 3B. Before surgery flexion of wrist was 46 degrees and extension was 45 degrees. Also before surgery grip strength was %62 less than in comparison to intact wrist. In radiological evaluation which was conducted before surgery we identified that 23 patients had negative ulnar variance, 6 patients had neutral variance and 2 patients had positive ulnar variance.
