Investigation of the relation of antibiotic susceptibility profiles of klebsiella pneumoniae i̇solates with capsule genotypes

One of the most important reasons for the frequent selection of Klebsiella pneumoniae is the factor that phagocytizes the bacteria and is the major virulence factor from serum bactericidal proteins. Another reason is β-lactamase enzymes for β-lactam immunity of bacteria. In our study, the possible relationship between the capsule genotypes and antibiotic susceptibility profiles of K. pneumoniae isolates isolated from various clinical specimens was investigated. To this end; Capsule genotypes (K1, K2, K5, K20, K54, K57) were determined by Polymerase Chain Reaction (PCR) method using specific primers in K. pneumoniae isolates isolated from clinical specimens. Antibiotic susceptibility phenotypically by disc diffusion and molecularly by PCR method for β-lactamase (ESBL) (CTX-M1, CTX-M2, CTX-M9, CTX-M 8/25, KPC, NDM-1, OXA-48) like) genes were investigated. In our study, 27 carbapenem-resistant K. pneumoniae isolates whose capsule genotypes were determined in a previous study in our department and 180 K. pneumoniae isolates isolated from various clinical samples whose capsule genotypes were to be determined were included in our study. Carbapenem resistant 27 K. pneumoniae from which we included in our study 1 isolate isolate K1, 5 isolate K2, 15 isolate K5, 4 isolate K20, 1 isolate K54, 1 isolate K57 while confirming that the capsule genotype belongs. In 180 K. pneumoniae isolates, 7 isolates K2, 3 isolates K20, 1 isolate K1 capsules genotype were detected. The CTX-M1 gene was detected in 3 isolates belonging to the capsular genotype K20. The NDM - 1 gene was identified in 3 isolates belonging to the capsular genotype K5 and 1 isolate belonging to K2. The OXA-48 like gene showed heterogeneous distribution among capsular genotypes in 24 isolates. CTX-M2, CTX-M9, CTX-M 8/25, KPC resistance genes were not detected in any isolate. Relationship of CTX-M1 gene with K20 capsule genotype, resistance relationship of AMC, TPZ, FOX, ETP antibiotics with K5 capsule genotype, sensitivity relationship of AMC, TPZ, IPM, TE, SXT antibiotics with K20 capsule genotypes were found to be statistically significant. As a result, we believe that the possible relationship between capsule genotypes and GSBL resistance genes will be better illuminated by studies that will include a regional and large number of isolates, and these studies will also shed light on future vaccine studies.
