Surgical treatment results of patients with developmental hip dysplasia aged 5 and over in our clinic

Introduction and Objective: The aim of this study is to investigate the long-term results of patients aged 5 years and older who were diagnosed with developmental dysplasia of the hip (DDH) and treated with Salter or Pemberton osteotomies in our clinic in the last 15 years. Materials and Methods: In this retrospective study, 95 hips from 74 sections between the ages of 5-9 years, known to have DKD, and radical reduction with Salter or Pemberton pelvic osteotomy were included. There were 27 patients in the Salter Pelvic Osteotomy group and 47 patients in the Pemberton Pelvic Osteotomy group. Before DKD treatment, Tönnis classification was distributed. All our patients were Tönnis stage 3 and stage 4 to learn the femoral head pressure reduction and femoral stability osteotomy to reduce the development of AVN. Preoperative, postoperative and final control Acetabular Index (AI), CE angles were measured. The presence of avascular necrosis in the femoral head reached the Ficat Arlet Staging system. Clinical and radiographic evaluation was performed according to the modified scoring system of Trevor et al. Results: When the correction rates of acetabular index were evaluated in our study, it was observed that Salter osteotomy provided more correction compared to Pemberton osteotomy; while the preoperative mean AI value was 43° in the Salter group, it decreased to 22° at the final controls and in the Pemberton group, these values ​​were measured as 40° and 24°, respectively. At the end of the long-term follow-up (average of 10 years), no statistically significant difference was observed between the two osteotomy groups in terms of AVN percentage according to FICAT stages, clinical evaluation and M. Trevor clinical scores and CE angle. Conclusion: Salter and Pemberton osteotomies can be safely used in the treatment of DDH considering their effects on the femoral head.
