Results of medical and surgical treatment in uveitic glaucoma patients

Purpose: Control of intraocular pressures with medical treatment in uveitic glaucoma cases, in the cases of inadequate medical treatment due to intraocular pressure increase caused by uveitis complications, surgical treatment options showing effects and treatment methods of the patients followed in our clinic. Material and Methods: 109 eyes of 85 patients with uveitic glaucoma were included in this study and the file records were evaluated retrospectively Results: Of the 85 patients, 41 were males (48.2%) and 44 were females (51.8%). It was found that 32 patients had right eye, 29 eyes had left eye and 24 patients had bilateral effected. Of the 109 eyes, 53 eyes (48.6%) had right eye involvement and 56 eyes (51.4%) had left eye involvement. The mean age of the patients ranged from 10 to 80 years with a mean of 44.6 ± 16.6 years. Patients were diagnosed with uveitic glaucoma for a minimum of 5 months and a maximum of 59 months (mean 33 ± 9 months). 86 patients were found to have 48 (44%) anterior, 23 (21.2%) moderate, 25 ) and 13 (11.9%) were in the form of panuveitis. 86 eyes (78.9%) from 109 eyes with uveitic glaucoma could be controlled by medical treatment. 23 eyes (21.1%) underwent glaucoma surgeries with visual loss and optic nerve pathology that could not be controlled by intraocular pressure despite maximum medical treatment. However, there was no statistically significant difference between steroid use, uveitis localization and surgeons. (P> 0.05) Conclusion: Uveitic glaucoma treatment depends on the underlying disease. Both ocular inflammation and optic neuropathy should be well monitored. For this reason, the form and route of treatment should be determined by a multidisciplinary team, including both uveitis and glaucoma specialists. In our study, 86 eyes from 109 eyes with uveitic glaucoma could be controlled by medical treatment and firstly beta-blocker combinations were selected. Surgical options include the need to use trabeculectomy with antimetabolites such as mitomycin or 5-fluorouracil and surgical treatment should be performed when uveitis is not active.
