ABSTRACT In recent years, the rate of delivery via caeserean section is increasing throughout the world. It's shown that incidence of complications; like plasenta previa, plasenta accreata, rupture of uterus and peripartum hysterectomy incrase with the number of previous caeserean sections. In our study, we compared incidence of caeserean sections, manegement and maternal fetal outcomes in women who have had four or more previous caeserean section with those in women with less order caeserean section. According to results, we aimed to estimate complications that might be occured and to decrease maternal and fetal morality and morbidity rates. Our study defined 635 women giving birth between 01.01.13 and 31.12.13 via caeserean section in Dicle University Faculty of Medicine, Obstetrics and Gynecology Clinique. 18 women that had multipl pregnancy or whose datas were not received excluded from the study. The 635 patients includued in the study were divided into two groups. Women who hae had one or two caeserean sections were identified in group 1 and those with three or high order previous caeserean sections identified in group 2. Women age ranged from 18 to 51 and mean maternal age was 31,28±5,21. The rate of placenta previa was 7.9 %, placenta accreata 6,3 %, rupture of uterus 1,6 %, maternal critical care admission 13%, blood transfusion 8% and preterm delivery 42% in group 2. This complications were less prevalant in group one. As a result, we concluded that women in group 2 have a significantly higher risk of maternal and neonatal complications than woman in group 1.
