Detection of the plazmid-mediated quinolone resistance determinant gene QNR among clinical isolates of Klebsiella pneumoniae

Abstract (EN)

A bacterium is known as a cause of lot of inflection in the community and is a mistake originated. Especially, in recent years with increasing ESBL producing strains, the treatment of the infections are made in difficult which are depending on K. pneumoniae. Quinolones have been in use since 1962, these are effective agents in both Gram-positive and Gram-negative bacteria. The result of DNA gyrase and topoisomerase IV enzyme inhibition, quinolones show bactericidal effect. Resistance quinolones in bacteria is usually the result of chromosomal genes mutations. However, in recent years, in addition to these mechanisms of resistance, there is another mechanism which is emerging, is called plasmid-mediated quinolone. These resistance genes are referred to as qnr. In recent years, increasing of plasmid-mediated quinolone resistance genes is constituted difficulties in the treatment of infections. The genes of qnr does though not cause alone to quinolone resistance, it causes to decrease in sensitivity of the quinolone and to increase in the minimum inhibitory concentration values. In our study, 90 ESBL positive and levofloxacin resistant were isolated from various clinical specimen, in the strain of K. pneumonia, aim to investigate the presence of genes which are qnrA, qnrB ve qnrS. The genes of qnr was investigated by real-time PCR method in strains which are included in the study. In the end of the study, 30 (33.3%) isolates qnrB gene, 5 (5.6%) isolates were positive for the gene qnrS. There was not qnrA gene in any strains. Plasmid can be transferred through qnr genes? increase in prevalence could prove importance in the terms of clinically. Therefore, the transmittable quinolone resistance should be monitor and determine the rates of disturubition, are important to identify the measures which should be taken.
