Comorbities and metabolic syndrome in COPD patients

Abstract (EN)

Objectives: In recent years, comorbities and metabolic syndrome in COPD (Chronic Obstructive Pulmonary Disease) and their association with the pulmonary function is commonly investigated. However differences between the methodology and the ethnical properties alters the datas. In our sudy, our goal is to research the frequency of the comorbidities and the metabolic syndrome which changes the prognosis and the treatment of the COPD between the patients with COPD and control group.Methods: Between November 2008-August 2009, the frequency of the comorbidities and metabolic syndrome were analysed in 142 patients with COPD diagnosed according to the GOLD (Global Initiative for Chronic Obstructive Lung Disease) 2006 and 71 control group with the similar age and sex who admitted to the Çukurova University pulmonary medicine outpatient clinic and Adana pulmonary medicine. Patients with COPD were >40 years, current or ex smokers and stable in condition. Comorbidities are divided into two groups; which one of is learned from the patients, the other the newly diagnosed by tests. Metabolic syndrome was diagnosed according to four clinical guide-line NCEP ATP III, IDF, EGIR and AACE.Results: At least one of the comorbidity is established at 90 % of the COPD patients and 77.5 % of the control group. The most commonly diagnosed comorbidities are; hypertension (41.5 %), metabolic syndrome (36.6 %), pulmonary hypertension (31 %), osteoporosis (28.9 %), coronay artery disease (20.4 %), depression (14.8 %), anemia (14 %), anxiety (12 %), diabetes mellitus (12 %). The prevelance of the pulmonary hypertension, osteoporosis and coronary artery disease in COPD patients were here and the increase was statistically significant. Prevelance of the metabolic syndrome diagnosed according to each of the 4 different clinical guideline was not stastatically significantly differant between the COPD patients and the control group (NCEP ATP III: 36.6 %- 42.3 %, IDF: 42.3 %- 45.1 %, EGIR: 19.7 %- 22.5 %, AACE: 27.5 %- 36.6 % respectively).Conclusions: In our study, comorbidities are common among the COPD patients. Longer studies are needed to objectively evaluate the comorbidities one of which is metabolic syndrome of COPD patients.Key Words: COPD, metabolic syndrome, comorbidity
