Effect of controlled hypotensive anesthesia on postoperative THROAT PAİN

Postoperative sore throat (POBA) is an important complication that is very commonly experienced after anesthesia and impairs the recovery quality of the patient. The incidence of POBA has been reported as 14.5%-50% after endotracheal intubation, and 14.4%-34% after the laryngeal mask usage. Controlled hypotension (CH) is a technique that is used to reduce the patient's blood pressure to a safe lower limit that will not impair the perfusion index, during surgery for clear vision, shorten the procedure duration. In this study, we aimed to investigate the effect of CH administration in patientswith fixed cuff pressure and supine position on POBA, cough, hoarseness, and vomiting. A total of 124 patients in the ASA (American Society of Anesthesiologists) Ⅰ-Ⅱ risk group, aged between 18-65 years who will undergo elective surgery, were included in the study. Patients diveded into two groups: Group N (n:65) normotensive patients and Group H (n:59) who CH is administrated. In both groups, the cuff pressure was adjusted to 25 cmH2O via a manual cuff meter. CH administration was provided with glyceryl trinitrate (nitroglycerin) infusion as the range of 55-65 mmHg mean arterial pressure (MAP). The patients were evaluated for POBA, cough, hoarseness, and vomiting during the postoperative period at the 15th minute, 2nd, 6nd, 12nd and 24nd hours. The groups were similar in terms of demographics, smoking, and ASA values. A statistically significant difference was found between the groups in sore throat NRS scores at the 15th minute and 2nd hour, and in hoarseness scores at the 2nd and 6th hours.Vomiting rates and cough scoreswere similar between the groups. In conclusion, we investigated the effect of controlled hypotension on the sore throat in the current study, and we found an increased incidence of POBA and hoarseness. Therefore, we thought that postoperative recovery quality may deteriorate in patientsfor who CH is implemented and should be attentive in patient selection.
