Abstract (EN)
Scientific studies have revealed the complications of blood use in cardiopulmonary bypass (CBP). Developing technology aims to reduce and eleminate the blood use in CBP applications. Hyperlactatemia is a common problem during CPB. Whether or not blood use has a hyperlactic effect is one of the arguments and it is thought to have an effect. The aim of this study is to investigate the effect of blood use on hyperlactatemia in coronary artery bypass surgery. In our study we retrospectively reviewed the patients who underwent coronary bypass surgery with extracorporeal system between January 2014 and December 2016 at Dokuz Eylül University Cardiovascular Surgery Clinic. Sixty patients included in the study were grouped with the blood usege under extracorporeal circulation (peroperatively) and those who were not use. We divided into Group 1: 30 adult patients without blood transfusion during coronary bypass surgery with extracorporeal circulation and Group 2: 30 adult patients with blood transfusion during coronary bypass surgery with extracorporeal circulation. Our study were not included the patients with cardiopulmonary bypass (CPB) duration of more than 3 hours, who had received urgent, no aortic cross-clamp (ACL), had infection and renal failure. In this study patient's preoperative,peroperative 30th, 60th, 90th minutes with CPB exit,intensive care unit arrival and postoperative 4th, 8th, 12th, 24th hours lactate (lct) and hematocrit (hct) levels were screened and analysed. As a result of patient analysis, CPB 30th and 90th minutes, CPB output, intensive care unit arrival and postoperative 24 hours hct values significantly a difference. In our study, it was found that blood transfusion had an effect on hyperlactatemia in CBP. While there was no differences between hct values, there was a significiant difference in the lct values in the same tine period.
