Can Serum NT-proBNP be used as a hypervolemia marker in patients with preserved ejection fraction and without advanced heart valve pathology?

Abstract (EN)
INTRODUCTION: It is extremely important to identify hypervolemia in hospitalized patients and to provide appropriate treatment. In this study, we evaluated NT-proBNP marker in hypervolemic patients with preserved ejection fraction and no significant valvular heart pathology. METHOD: Our study was conducted by evaluating patients hospitalized with a preliminary diagnosis of hypervolemia in March and August 2023. Patients with an ejection fraction below 55% and severe valve disease, undetermined hypervolemia, under the age of 18, and intake and output could not be monitored were not included. Patients' age, gender, comorbidities, follow-up of their intake during the hospitalization period, total fluid balance, pathological findings of hypervolemia in physical examination, serum NT-proBNP, creatinine albumin, sodium levels, hemoglobin laboratory parameters, lung x-ray, thorax tomography and echocardiography findings were recorded. The collected data were evaluated using statistical methods RESULTS:69 hypervolemic patients were included in the study. Control NT-proBNP parameter of patients which hypervolemia treated, showed a statistically significant decrease compared to the NT-proBNP values measured before treatment (p:0.012) It was determined that there was a significant difference between the initial and subsequent NT-proBNP values of patients whose hypervolemia increased after hospitalization. An increase in NT-proBNP values was detected in patients which fluid balance remained positive. Statistically significant correlations were detected between serum NT-proBNP values and serum creatinine, sodium, albumin and blood hemoglobin values. CONCLUSION: NT-proBNP parameter should be considered as a marker of hypervolemia, and monitoring this marker at regular intervals can give an idea about the volume status in patients with hypervolemia with preserved cardiac ejection fraction and no obvious heart valve pathology.
