Abstract (EN)
This study examined the level of chronic disease management among healthcare professionals working in Family Health Centers (FHCs) and explored its relationship with workload, job control, and social support. The study population consists of 47 Family Health Centers (FHCs) located in the central district and counties of Yozgat. Out of approximately 300 healthcare professionals, a minimum sample size of 169 was determined with a 95% confidence level and 5% margin of error, and data were collected from 174 participants. The data collection tools included the Assessment of Chronic Illness Care (ACIC) Scale and the Swedish Demand-Control-Support Questionnaire. Data were analyzed using SPSS 25.0 with descriptive statistics, t-test, ANOVA, and regression analysis. Most participants were aged between 31–40, held a master's degree, were married, had 11–20 years of professional experience, and reported a balance between income and expenses. Demographic variables such as gender, age, and professional experience were found to have significant effects on chronic disease management. According to the regression analysis, social support had the strongest and most significant positive effect on chronic disease management, while workload and low job control negatively affected it. The findings indicate that chronic disease management is closely related not only to clinical but also to organizational and psychosocial factors; therefore, balancing workload, increasing areas of job control, and strengthening social support for FHC staff are essential for effective chronic disease management.
