Detection natural lamuvidine resistance by inno-lipa in patients with chronic hepatitis b

Two billion individuals worldwide have exposed to hepatitis B virus (HBV) and 350 millions of them are chronically infected with HBV. Infection experienced in newborn period and during the first one year of life becomes chronic in the ratio of 90%, this ratio reduces to 30% between 1-5 years. Risk of chronicity is about 2-5% in adult age. Main routes for HBV transmission are vertical transmission, parenteral transmission, horizontal transmission, unprotected sexual intercourse and nosocomial transmission caused by medical procedures. Pegylated-interferons with immunomodulatory and antiviral effects and oral antiviral agents are used today for treatment of chronic hepatitis B. Of oral antiviral agents, lamivudine blocks HBV replication providing enzyme, reverse transcriptase (RT) and prevents viral replication. In this study, it was aimed to detect primary resistance to lamivudine which is a drug often used today for chronic hepatitis B treatment and assess effectivity and suitability of initiation lamivudine treatment.One hundred patients who were admitted to Clinical Microbiology and Infectious Diseases Clinic of Fırat University Hospital and who did not receive any antiviral therapies evaluated for investigate lamivudine resistance. We detected 78 patients HBeAg negative and 22 patient HBeAg positive. We detected statistical significance correlations with HBeAg positivity, elevated ALT levels and HBVDNA levels (p<0,05). We used İNNO-LİPA test for investigate primary lamivudine resistance. We observed rtM204V and L180M mutations only one patient who have HBe positivity.It was concluded that the hepatitis B virus which is our region is current virus and lamivudine is effective and suitable drug for treatment of chronic hepatitis B.Key Words: Chronic hepatitis B, Lamivudine, Primary resistance, Innolipa
