Preoperative and postoperative psychiatric evaluotion of patients with chronic otitis media

ABSTRACT Aims: In this study, we aimed to compare the quality of life and mental health of patients with chronic otitis media pre-and postoperatively. Patients with chronic otitis media may have symptoms like hearing loss, ear discharge, tinnitus and vertigo which can affect social and physical condition. Changes in these symptoms may affect the psychiatric problems of patients after surgery. Materials and Methods: Our research was conducted on 50 patients who were operated for chronic otitis media between November 2012 - May 2013 in Dicle University Medical School Department of Otorhinolaryngology. Comparison of psychological symptoms and quality of life profile was done by using Symptom Check List 90-Revised Form (SCL-90-R) and Short Form 36 (SF-36) scale. These tests were fille by the patients before surgery and 6 months after surgery. Results: Fifty patients were involved in study. The mean age of patients was 26,1 years and the age range was between 18-55. The only statistically significant change in SF- 36 was bodily pain postoperatively. When the SCL-90- R test was evaluated, statistically an improvement was observed in obsessive-compulsive, interpersonal sensitivity, anxiety, anger - hostility, and global severity index postoperatively. Thirty six patients among 50 patients showed improvement in hearing levels. Evaluation of SF-36 test of this 36 patients showed statistically significant improvement in bodily pain, general health perception and mental health parameters postoperatively. In SCL -90- R test, obsessive-compulsive, interpersonal sensitivity, anxiety, anger - hostility, and global severity index was found to be improved statistically. Also, 36 of 50 patients who underwent surgery had additional symptoms other than hearing showed improvement. In this group, SF-36 demonstrate significant improvement in bodily pain and mental health parameters. Additional evidence of improvement in parameters of 36 patients were obsessive-compulsive, interpersonal sensitivity, depression, anxiety, anger - hostility, and global severity index when the was SCL -90- R was evaluated. Conclusion: Patients with may cause changes in quality of life and psychological state. Psychiatric aspect of chronic ilnesses should be dealed by combination of clinicians with psychiatrics to get better results. Health- related quality of life V measures in chronic otitis media, as in many diseases play an important role in treatment efficcacy of this disease.
