Özet (EN)

Comparison of Fibrosis Degree and Non-invasive Fibrosis Scoring Methods in Biopsied Patients with Chronic Viral Hepatitis B, C and Non-Alcoholic Fatty Liver Disease. Eskişehir Osmangazi University Faculty of Medicine Internal Medicine Specialization Thesis, Eskişehir, 2024. In our retrospective study, we compared the degree of fibrosis with non-invasive fibrosis scoring methods in patients diagnosed with Chronic Viral Hepatitis B, Chronic Viral Hepatitis C, or Non-Alcoholic Fatty Liver Disease who underwent biopsy. A total of 381 patients who presented to our clinic were included in the study, including 183 with Hepatitis B, 193 with Hepatitis C, and 5 with NASH. Patients were divided into two groups based on the Ishak fibrosis staging system: mild fibrosis (F1-2-3) and advanced fibrosis (F4-5-6). In both groups, we calculated the values for APRI, FIB-4, platelet count, MPV, AST, ALT, total bilirubin, direct bilirubin, and albumin. We compared these calculated markers between the two groups in terms of their ability to distinguish advanced fibrosis. In Hepatitis B, APRI, FIB-4, platelet count, age, AST, total bilirubin, and albumin were found to be statistically significantly higher (p<0.05). MPV, ALT, and direct bilirubin were not found to be significant. The AUROC values for APRI, FIB-4, platelet count, AST, total bilirubin, and albumin were found to be 0.712, 0.807, 0.777, 0.651, 0.706, and 0.762, respectively. In Hepatitis C, APRI, FIB-4, platelet count, AST, total bilirubin, direct bilirubin, and albumin were found to be statistically significantly higher (p<0.05). MPV and ALT were not found to be statistically significant. The AUROC values for APRI, FIB-4, platelet count, AST, total bilirubin, direct bilirubin, and albumin were found to be 0.739, 0.853, 0.780, 0.706, 0.653, 0.641, and 0.708, respectively. Due to insufficient cases in the NASH group, statistical analysis could not be performed. FIB-4 was found to be the best diagnostic test in both Hepatitis B and Hepatitis C groups.
