Istanbul One of the best treatment option for replacing missing teeth is dental implants and survival rates of dental implants vary depending on many factors such as surface properties of implants, patient habits, prosthetic factors and bone density. Bone density is a determining factor in dental implant treatments especially in short dental implants. The literature showed that implant losses were more experienced in areas with low bone density due to peri-implant forcing and tensions. The purpose of this study was to compare primary stability (ISQ values), the insertion and extraction torque values of 5-mm short implants and 10-mm standard size implants into two different types of bones (D2 and D4 bone densities). A total of 40 dental implants 4.3 mm diameter and 5 mm (n=20) and 10 mm (n=20) in length were placed into the same polyurethane bone block which imitates the structure of D2 and D4 bone type. While inserting and removing the implants, the torque values were recorded at each iteration, and the mean values were calculated from start to finish. The torque values for insertion were calculated until the implants reached the bone level. Primary stability was measured after implant insertion using (RFA). SPSS was used to analyse the data. Results of this study showed that the insertion torque value of 5 mm length implant in D2 bone (35±SD) was not statistically lower than the insertion torque value of 10 mm length implant in D2 bone (36.5±SD). However, the insertion torque value of 5 mm length implant in D4 bone (2±SD) was found to be statistically lower than the insertion torque value of 10 mm length implant in D4 bone (3±SD). The removal torque value of 5 mm length implant both in D2 bone (15.5±SD) and D4 bone (6.3±1.05) was not statistically lower than the removal torque value of 10 mm length implant in D2 bone (18±SD) and in D4 bone (6.7±1.15). However, the ISQ value of 5 mm length implant in D2 bone (50±19.04) and in D4 bone (41.8±8/84) was found to be statistically lower than the ISQ value of 10 mm length implant in D2 bone (66.5±2.22) and in D4 bone (54.5±2.06). The current study concluded that the 5-mm implants may be used as an alternative to augmentation techniques in the mandibular posterior region, but they were not reliable in the maxillary posterior region. Further studies with larger samples and prospective clinical studies are necessary to validate the current findings. Key Words: Implant Stability, Mandibula, Maxilla, Insertion and Extraction torque
