The effectiveness of emergency department observation on hospitalization and mortality for blunt multi trauma patients

Abstract (EN)

Today despite of social and economical development, trauma is the most important public health problem for all countries. Although young people are faced with trauma more than old people, old people mortality rates are more than young ones. Nevermore 78% of mortality reason for ages between 15 and 24 is trauma. Regional trauma patients apply to our level three trauma center emergency department. Our aim is to overview patients with blunt multi trauma mechanism that applied our emergency department retrospectively within their treatment period in emergency department or other surgical departments and parallelism to today?s conservative approach.In our study we evaluate 226, over 15 age patients that applied to Dicle University Emergency Department between January 2006 and January 2008 due to blunt multi trauma such as motor vehicle crush, fall etc. retrospectively according to their definitive medical records. For statistical analyze we divide our patients two groups, treated in emergency department (n=105), treated in surgical departments (n=121). For our 226 patients 71.7% (n=162) of them were male and 28.3% (n=64) were female. Nineteen (8.4%) of trauma patients have died. Age average for all patients was 35.9±15.882 (15-94). Blood transfusion requirement effected to surgical department transfer for patients (p=0.00). Between our groups alanin transaminase, aspartate transaminase, hematocrit and white blood cells were scientifically for statistical analyze (p>0.05). Injury Severity Score and Abbreviated Injury Scale (AIS) were available for group 2 patients scientifically (p<0.05). Four of our patients were pregnant and they treated at surgical departments (p<0.05). In physical examinations for abdomen sensibility and defense were significant for hospitalization (p<0.05).Multivariate analysis showed that high white blood cell levels (OR=1.0, CI=1.00+1.00, P<0,01), high AIS score (OR=4.17, CI=2.34+7.43, P=0,00) and blood transfusion requirement (OR=0.042, CI=0.005+0.33, P<0,01) were effective for hospitalization.
