Lumbar disc herniation which neurosurgeons mostly meet in comes on front lines of ilnesses that cause labor and financial loss. Lomber disk herniation is seen with patients who are young or middle ages 70 percent of cases is up 30-50 ages and 30 percent of them is counting after 60s ages. Pain and other symptoms occurs depending on irritation of sensitive structures. The origin of lumbago is posterior part of annulus fibrosus, posterior longitudinal ligaman (PLL), faset arthrosis capsule and periost. Annulus fibrosus ruptures with herniation and PLL stretches. The reason that occurs lumbago with lomber disk herniation is effects on PLL and annulus fibrosus (40).15 percent of patients has surgery in spite of all treatments. 286 patient operated with flaviotomy and microsurgery method in last year were observed. Oswestry Pain Scala was applied to patients preoperative and in third month of postoperative period. Significant healing was sighted postoperative. Furthermore, healing in patients? preoperative and postoperative Oswestry Scalas were observed as significant improvement.Lumbar spinal stenosis is frequently reason of lumbar and lower extremity pains in older ages. Patients with spinal stenosis are people who are old and have lumbago for a long time. Lomber spinal stenosis? symptoms and signs on patients appear on conclusion of changements in spinal canal, radicle canal, bone and connective tissue around radicle canal, degeneration of disc, ligaman, faset hypertrophy and osteofit forms. Life quality of patients applied with lumbago, lower extremity pain, neurogenic cladication and sphincter most commonly decreases. On cases which conservative treatment doesn?t utilize, surgery decompration of radicle press is obligated. Appearance instability signs on patients after total laminectomy operation a classic method on spinal stenosis and after these operations increase of some complications on these old patients, orientate surgeons apply minimal invasive surgery technique. Bilateral microdecompration with one sided approach that practiced in our clinic is a method which has little complication, obtains early mobilization of patients and doesn?t damage stability. At this study 49 case were observed prospectively. Oswestry Pain Scala was applied to patients preoperative and in third month of postoperative period. Significant healing was sighted postoperative.
