Abstract (EN)
Suicide emerges as an important public health problem with its increasing importance day by day. It is promising to develop many models to explain the multifactorial complex nature of suicidal thought and behavior. Understanding the nature of suicidal thoughts and behaviors is essential to reducing suicide and related conditions. In our study, it was aimed to compare patients with major depressive disorder with suicide attempt, major depressive disorder without suicide attempt and healthy controls in terms of metacognitions and rumination-related metacognitions. A total of 150 patients, including 50 patients with a diagnosis of major depressive disorder with a suicide attempt, 50 patients with a diagnosis of major depressive disorder without a suicide attempt, and 50 healthy individuals without any mental illness, were included.Sociodemographic and clinical data form, Hamilton Anxiety Rating Scale (HAM-A), Montgomery and Åsberg Depression Scale (MADRS), Metacognition questionnaire-30(MCQ-30), Cognitive Attention Syndrome-1 Scale (CAS-1), Suicide Cognitions Scale(SCS), Positive Beliefs about Rumination Scale(PBRS), Negative Beliefs about Rumination(NBRS) Scale were administered to all participants. When the groups were compared in terms of depression and anxiety levels; Depression and anxiety levels were higher in the patient group than in the control group, and this difference was statistically significant.Among the scales and subscales comparing metacognitions, the patient groups had higher scores than the control groups. NBRS total score of patients with suicide attempt, negative interpersonal and social consequences of rumination which (NBRS-2) is NBRS subscale, time devoted to rumination and threat monitoring, MCQ-30 total score, uncontrollability, need to control thoughts subscale scores, ıt was found that there was a significant difference from the depression group without suicide attempt. There is a significant difference between the patient groups and the control groups in the PBRS, the uncontrollability and harmfulness of rumination (NBRS1), cognitive lack of confidence and danger (MCQ3), and cognitive awareness (MCQ5) scales. There was no difference between the patients with suicide attempt and the patients without suicide attempt. When evaluating the correlations of the scales between the depression groups with and without suicide attempt, a weak positive correlation was found with the uncontrollability of rumination (MCQ2) and the need to control thoughts (MCQ4) with MADRS in the depression group with suicide attempt. A moderate positive correlation was found with the interpersonal and social consequences of rumination (NBRS2), NBRS, and suicide cognition scales. In the depression group without suicide attempt, different from the group with suicide attempt, a moderate positive correlation was found with MADRS in cognitive awareness and MCQ-30 scores. There was no correlation between PBRS and MADRS scores of both groups. With the findings of this study, it is thought that understanding the place of metacognition and related beliefs, especially in patients with suicide attempt, may contribute to the use of metacognitive therapy, which is an emerging treatment method, in clinical interventions.
