Abstract (EN)
ABSTRACT Introduction Melanoma that develops from melanocytes in the basal layer of the epidermis is the most dangerous malignant skin tumors and in recent years increases rapidly. Today melanoma patients follow with serum markers such as S100 and LDH in many cases to show the progression of the disease, but in some conditions like hemolysis, hepatitis, myocardial infarction, muscle disease, infections, etc. can cause problems in follow up and make false positive. The aim of this study is a research new marker of responding to needs that can be used melanoma early diagnosis or clinical follow-up, patients in different melanoma stages determine serum LDH, CRP, S100 levels SAA and Lp-PLA2 levels. Materials and Methods In this study, Ege University dermatology department admitted total of 131 melanoma diagnosed patients (stage I = 33, stage II = 32, stage III = 31, stage IV = 29, 3 mucosal, 2 uveal and 1 melanoma insutu ) and 27 healthy volunteers taken blood samples and studied LDH, CRP, SAA levels and Lp-PLA2 activity. Findings In the patient group compared to the control group Lp-PLA2 activity, LDH, CRP SAA, S100 levels were significantly higher. In the patient group compared to the control group, average level of S100 in stage III and IV and the level of LDH and CRP were higher in all stages.This elevation is only found to increase with the increase of LDH stage. The average level of SAA and Lp-PLA2 activity compared to the control group were increase from stage I and increase in value on stage I, II, IV which continued to increase and in stage III a relatively high level observed to be maintained. In control group and different stages statistical evaluation detected significant difference found between Lp-PLA2 from stages I and II and between I to IV, S100 stage I to III, in stage II and III, LDH in control group and stage II, III and IV, SAA I, II, III, IV and CRP just between the control group and stage II. The stages in the patient group, S100, LDH, CRP, SAA levels and Lp-PLA2 activity receives the assessment as Spearman's correlation analysis of the relationship between the values rho (r) coefficient, respectively, 0.408, 0.360, 0.387, 0.622 and was found to be 0. 361, and the highest for correlation SAA respectively. The sensitivity (95%) and specificity (93%) of SAA were identified as the highest hence the most valuable marker. Also there were significant positive correlation between the level of SAA with Lp-PLA2 activity. Conclusion In this study, for the first time in literature Lp-PLA2 activity which is an inflammatory and SAA levels in different stages of malignant melanoma has been shown to increase in correlation. The inflammatory and immunological mechanisms are not yet understood in development and progression of melanoma, and considering the SAA levels and the rise of Lp-PLA2 enzyme activity, is an important indicator of the transition to the next stages can be used in clinical practice.
