Abstract (EN)
Introduction and Aim: Patient might have different presentation due to mushroom poisoning. Some cases might have renal injury. It is very important to determine acute renal injury early phase. The aim of our study is to determine the utility of Neutrophil Gelatinase Associated Lipocalin as a biomarker in predicting renal injury in the early phase of the patients aged oner 18 with complaints due to mushrooms Materials and Methods: The patients who were admitted to the Cukurova University Medicine School ED between 11th September 2017 and 31th May 2019 with complaints after eating mushroom who over 18 years old and have the inclusion criteria and given written consent were included in the study. The demographic chharasteristic, past medical history, complaints and physical findings of the patients were evaluated beside biochemical and hematologic test urine and serum NGAL on the 0. and 6. Hours and 3. Day of the patients were meausred. 47 cases were included. Results: There were 24 (51%) males. The mean age was 49.83±18.40 (18-84 ages) in the males, 41.17±15.05 (20-69 ages) in the females. Most common complaints were nauseau (39, 83%), vomitting (39, 83%) and abdominal pain (21, 44.7%). Mean hospitalisation time was 1.28±0,61 (1-3 days) days. Mean urine NGAL 0. and 6. hours were 379.57±397.57 and 421.03±506.71. Mean plasma NGAL 0. and 6. hours were 1116.17±762.47 and 1260.86±1103.13. WBC 0. hour and NGAL serum 0. and 6. hour measurement were statictically significant (0. hour p=0.009, 6. hour p=0.017). NGAL 0. hour and platelet 0. and 6. hours measurement were statictically significant (0. hour platelet p=0.013, 6. hour platelet p=0.008). Urine NGAL 0. hour and delta creatinin 0-3 measurement were statictically significant (p= 0.052). Serum NGAL 6. hour and delta creatinin 0-3 measurement were statictically significant(p=0.035). There was weak correlation between urine 0. and 6. hour urine NGAL(p=0.005), intermediate correlation between delta creatinin 6-3 and 0-3(p=0.000), weak correlation between serum 0. and 6. hours NGAL(p=0.000), intermediate correlation serum 6. hour NGAL and delta creatinin 0-3(p=0.026). Conclusion: Elevated 0. hour urine and 6. hour serum NGAL levels might predict elevation of serum creatinin on the 3. day. For this reason, NGAL maybe used as a biomarker in patients over 18 years old who may have renal injury due to mushrooms. NGAL assays maybe affected with WBC and platelet count.
