Relations of clinical parameters and effects on prognosis of genomic polymorphisms in NOS3 and TNF alpha in patients with multiple myeloma

Abstract (EN)

In this study, relations of clinic parameters and effects on prognosis of genomic polymorphisms in NOS3 (+894, VNTR) and TNF? (-308, -238 and -857) are assessed in patients with multiple myeloma. Seventy seven patients with multiple myeloma in Haemotology department in Internal Medicine of Gaziantep University Hospital are included. Healty 77 people without any heamotological malignancies are icluded as control group. Distribution of NOS3 (+894) gene polymorpism is GG: 37 (48.1%), GT: 25 (32.5%), TT: 15 (19.4%), distribution of NOS3 (VNTR) gene polymorpism is AA: 40 (51.9%), AB: 28 (36.4%), BB: 9 (11.7%), distribution of TNF? (-308) gene polymorpism is AA: 0, AG: 18 (23.4%), GG: 59 (76.6%), distribution of TNF? (-238) gene polymorphism is AA: 0, AG: 13 (16.9%), GG: 64 (83.1%) and distribution of TNF? (-857) gene polymorphism is TT: 16 (20.8%), TC: 28 (36.4%) CC: 33 (42.8%). No significant relation is found between TNF? (-238 and -857) gene polymorphisms and MM. GG genotype in TNF? (-308) gene polymorphism (p=0.012) and TT genotype in NOS3 (+894) gene polymorphisms (p=0.004) are significantly increased in patients with MM in comparison with control group. However AA genotype in NOS3 (VNTR) gene polymorphism (p=0.007) and GG genotype in NOS3 (+894) gene polymorphisms (p=0.004) are significantly decreased in patients with MM incomparison with control group. As a result, TT genotype in NOS (+894) polymorphism and GG genotype in TNF? (-308) polymorphism which related to decreased expression are increased in patients with MM, thus, this may play a role in etiopathogenesis of multiple myeloma. Moreover, GG genotype in TNF? (-238) gene is not affected whatever the first-line therapy (OPKHT, bortozomibe, thalidomide) is, but related with early progression not affecting total survival.
