Prognostic importance of neutrophil lymphocyte rate and thrombocyte lymphocyte rate in multiple myeloma patients

Background and aim: Multiple myeloma (MM) is the second most common hematological malignancy seen after nonhodgkin lymphoma. It is characterized by 10-15% of all hematological malignancies. The aim of this study was to evaluate whether the neutrophil and lymphocyte counts at the time of diagnosis can be used as a predictor for the survival of the lymphocyte counts. Materials and Methods: The study was conducted between January 2012 and September 2018 in Şahinbey Research and Application Hospital. Multiple Myeloma diagnosis was made according to International Myeloma Working Group (IMWG) criteria. Spearman rank correlation analysis and ROC analysis were used to test the relationship between numerical variables. Findings: Neutrophil / lymphocyte ratio was found to be with creatinine (p: 0,010), age of diagnosis (p: 0.011), ECOG of 2-4 (p: 0,044), female gender (p: 0,002), ISS stage 2 (p: 0,002). showed correlations with survival (p: 0,040) in patients receiving immunomodulatory and proteosome inhibitors. Each unit increase in the neutrophil / lymphocyte ratio increases the risk of death by 1,093 fold (HR: 1,093% 95 CI: 1,018-1,173, P: 0,015). In our study, a value of 1,7 was predicted as a cut-off value for the prognosis of neutrophil / lymphocyte ratio, but we found that this value was clinically weak, whereas the platelet / lymphocyte ratio was not significant for prognosis. Results: In order to evaluate the prognosis of multiple myeloma, we found that Neutrophil / Lymphocyte ratio did not provide a prediction in addition to systems such as ISS and R-ISS. We suggest that new data should be used to determine whether the cut-off points obtained by the proportion of biochemical and hematological laboratory values within the risk factors in MM are important for prognosis.
