Abstract (EN)
Chronic rhinosinusitis with nasal polyps is a chronic inflammatory disease affecting the nasal cavity and paranasal sinus mucosa, and although many studies have been conducted, its pathophysiology is not yet fully known. In this study; considering the genetic background of nasal polyposis, whose etiopathogenesis has not been clearly elucidated, and its relationship with asthma, ORMDL3 and LRRC3C gene polymorphisms and serum IFN-γ, IL-4, IL-5 and IL-13 levels were examined, and their roles in nasal polyposis were evaluated. Nasal polyposis and healthy controls who applied to Fırat University Medical Faculty Otorhinolaryngology outpatient clinic were included in the study. Group 1: eosinophilic nasal polyps diagnosed with asthma, group 2: eosinophilic nasal polyp diagnosed without asthma, group 3: non-eosinophilic nasal polyp diagnosed and without asthma, group 4: healthy individuals. When the groups were compared in terms of ORMDL3 gene rs7216389 polymorphism, a significant increase was observed in the wild type allele in group 1 compared to the control group (for genotypes; p = 0.002, OR: 4.023 (1.227-13.191, for alleles p = 0.046, OR: 1.844 (1.048- 3.244)). ). LRRC3C gene rs3744246 polymorphism did not differ significantly between groups and when compared with the control group (p> 0.05). In the comparison between the genotypes of the ORMDL3 and LRCC3C polymorphisms and serum IFN-IL, IL-4, IL-5 and IL-13 levels, no significant relationship was observed between the groups (p> 0.05). In our study, serum levels of IL-5 and IL-13, which play a role in the development of eosinophilic nasal polyposis and non-eosinophilic nasal polyposis, were found to be high, while serum IFN-γ levels were observed to be low. IL-4 level was found to be significantly higher in the nasal polyposis group with asthma. In terms of gene polymorphism, we found that ORMDL3 gene polymorphism is effective in etiopathogenesis in patients with eosinophilic nasal polyposis with a diagnosis of asthma. No gene polymorphism was observed in eosinophilic nasal polyposis and non-eosinopic nasal polyposis without a diagnosis of asthma.
