Retrospective evaluation of the effect of preoperative TSH values and thyroid autoantibody levels on the development of thyroid cancer in patients with thyroidectomy between 2013-2018 in KTU Farabi Hospital DUE to noduler thyroid disease

Thyroid cancer is the most common malignancy of the endocrine system and its prevalence continues to increase. More than 90% of thyroid cancers are differentiated thyroid cancer. Papillary thyroid cancer is the most common thyroid malignancy world wide and has a good prognosis. In some studies, serum TSH elevation was found to be associated with thyroid cancer. The disease most commonly associated with thyroglobulin antibody (Anti-Tg) and thyroid peroxidase antibody (Anti-TPO) is autoimmune thyroiditis (Hashimoto's disease). Increased risk of thyroid cancer in patients with Hashimoto's thyroiditis. The aim of this study is to investigate the relation of these parameters to thyroid cancer by examining preoperative TSH, Anti-Tg and Anti-TPO values in patients with thyroidectomy in the K.T.Ü. Farabi Hospital. In our study, the data of 1909 (1490 female, 419 male) cases who had undergone total thyroidectomy operation in K. T. Ü. Farabi Hospital between 1 January 2013 and 31 December 2018, and were pathologically diagnosed with thyroid cancer or nodular goiter were analyzed retrospectively. In the study, papillary thyroid cancer was the most common thyroid cancer type in patients with thyroidectomy in K.T.Ü. Farabi Hospital. 79% of the patients with thyroid cancer were female. Thyroid cancer was detected in 63.4% of the cases whose preoperative TSH was higher than the reference range. Thyroid cancer was detected in 60.7% of the patients with preoperative Anti Tg positivity and in 46.1% of the patients with preoperative Anti TPO positivity. The relationship of preoperative TSH and Anti-Tg with thyroid cancer was statistically significant, but the relationship between preoperative Anti-TPO and thyroid cancer was not statistically significant. It was found that the frequency of thyroid cancer increased in patients with high preoperative TSH level and thyroid autoantibody positivity. However, more extensive studies are needed on this subject.
