2. ABSTRACT The Effect Of Bosentan Which Is A Non-Selective EndotheJin Receptor Antagonist On Intestinal Damage And Anastomotic Healing In An Experimental Crohn's Disease Model In the intestinal tissues of patients with Crohn's disease, high endothelin immunoreactivity and vascular abnormalities that can be cause to ischemia. We aimed to evaluate the effect of endothelin antagonism on healing of small intestinal anastomosis in an experimental Crohn's disease model. Twenty eight Spraque-Dawley rats were divided into 4 groups. Rats were treated with bosentan every day starting two days prior to induction of inflammation. Group I was sham operated. Inflammation was induced in Group III and Group IV with intrajejunal iodoacatemide administration. Only vehicles were given to Group II, as a control group. Three days later proximal half of same jejunal segment was resected and end-to-end anastomosis was performed in all rats. Four days after second procedure relaparotomy performed. Peritonitis scores and anastomosis bursting pressures were evaluated and then tissue samples were taken for histopathological analysis and hydroxyproline assay. Crohn's like inflammation could not formed in all samples. Severity of acute inflammation was less in bosentan treated group (pO.OOl). Mean tissue hydroxyproline level of Group IV was significantly higher than Group III (p<0.001). Although mean anastomosis bursting pressure of was less than those Group I and Group II, it was higher than Group III (pO.OOl). Mean peritonitis score of Group III (15.50±3.99) was significantly higher than those of others (pO.OOl). xnAs a result, the blockade of endothelin receptors reduced the severity of acute intestinal inflammation induced with iodoacetamide and improved anastomotic healing.
