Effect of blood pressure pattern on oxidative stres parameters in normotensive patient

Abstract (EN)

Purpose: The aim of this study was to investigate the effects of different blood pressure patterns (especially dipper, non-dipper and reverse dipper groups) observed in normotensive individuals on oxidative stress parameters. Materials and Methods: A total of 127 healthy normotensive individuals aged 21-68 years, without systemic disease who applied to the Department of Cardiology at Afyonkarahisar Health Sciences University, were included in the study. All participants underwent 24-hour ambulatory blood pressure monitoring (ABPM). Based on their daytime and nighttime blood pressure averages, individuals were divided into three groups: dipper, non-dipper and reverse dipper. Oxidative stress parameters such as Total Oxidant Level (TOS), Total Antioxidant Level (TAS), and Oxidative Stress Index (OSI) were measured spectrophotometrically in venous blood samples taken from the participants. Findings: As a result of the analyzes, TOS averages were calculated as 34.63 μmol/L in the dipper group, 73.67 μmol/L in the non-dipper group, and 85.23 μmol/L in the reverse dipper group. TAS averages were calculated as 106.00 μmol/L in the dipper group, 39.59 μmol/L in the non-dipper group, and 45.86 μmol/L in the reverse dipper group, and OSI averages were calculated as 22.05 μmol/L in the dipper group, 84.74 μmol/L in the non-dipper group, and 86.32 μmol/L in the reverse dipper group. In the analyzes, it was determined that OSI and TOS levels were statistically significantly higher in the dipper and reverse dipper groups, while TAS levels were significantly lower (p<0.001). Oxidative stress markers were particularly significantly impaired in the reverse dipper group. Furthermore, significant correlations were found between age, body mass index (BMI), and lipid profiles (LDL, HDL, and TG) of the individuals and some oxidative stress parameters. Strong correlations were found between Holter measurements and TOS, TAS, and OSI values; OSI values were observed to increase as nocturnal blood pressure decreases. Conclusion: The findings reveal that circadian blood pressure patterns have a significant impact on oxidative stress levels in individuals. Oxidative stress has been shown to increase significantly in non-dippers, and particularly in reverse dippers, despite being normotensive. This suggests that these individuals may be at a subtle but progressive risk for cardiovascular disease. Therefore, pattern analysis using ambulatory blood pressure monitoring, not only in hypertensive patients but also in normotensive individuals, may be an effective tool for the early identification of potential risk groups.
