The relation between endothelial dysfunction and insulin resistance in obes and non obes patients with polycystic ovary syndrome

Abstract (EN)

Objective: Visfatin is a new adipocytokine, which has a role in obesity, insulin resistance and inflammation. It is controversial in previous studies that body mass index may be related with serum visfatin levels in polycystic ovary syndrome (PCOS). This study was performed to investigate the relation between endothelial dysfunction and serum visfatin levels and insulin resistance among both obese and nonobese patients with PCOS.Material and Method: The study was perfomed on 58 patients with PCOS, of whom 13 were obese and 45 were non-obese, and age matched 13 obese and 39 non-obese healthy volunteer women. Fasting blood glucose, fasting insulin, FSH, LH, estradiol, prolactin, TSH, total testosteron, 17 OH progesteron, free testosteron, DHEAS, lipid profile, biochemical parameters, serum visfatin, ADMA and CRP levels were measured in the blood samples of all subjects between the second and the fifth days of the menstrual cycle.Results: Serum visfatin levels were found to be higher in both obese and nonobese PCOS patients than the healthy volunteers and not related with obesity. But, in obese PCOS patients insulin resistance was higher than nonobese PCOS patients. Serum ADMA level didn?t differ among PCOS patients compared to control group and wasn?t related with any antropometric and metabolic parameter. Serum visfatin level was positively correlated with serum HDL cholesterol, but negatively correlated with serum TG levels. Besidez, although there wasn?t any relation between serum visfatin level and HOMA, there was a negative correlation between visfatin level glucose/insulin ratio.Conclusion: In patients diagnosed as PCOS, either obese or non-obese, endothelial dysfunction found to be not related with insulin resistance and serum visfatin levels. Further studies, performed in larger populations using direct measurement methods, are needed in order to show that high visfatin levels may
