Özet (EN)

Purpose: Insulin resistance is defined as a decrease in the response of target tissues to insulin in response to normally present or increased insulin levels in the peripheral circulation. At the same time, insulin resistance is a clinically significant abnormality that can progress to diabetes. Some precautions can be taken and the examination and treatment can be made more effective by using methods that can detect people at high risk in terms of insulin resistance. Due to the limited scope of examination, especially in primary health care institutions, formulas that will calculate insulin resistance from traditional obesity body measurements and existing blood examination parameters will make significant progress in predicting these high-risk patients in the population. In this study, we primarily examine the relationships between waist circumference (WC), body mass index (BMI), waist/hip ratio (WHR), waist/height ratio (WHR) measurements and insulin resistance (ID) calculated with the HOMA-IR index formula in obese people. We wanted to examine it. We wanted to examine how insulin resistance in these people is affected by the increase and decrease in WC, BMI, waist/hip ratio, waist/height ratios, and whether there is a correlation or relationship when Homeostasis Model Assessment (HOMA) and Triglyceride Glucose indices are compared. Thus, we think that this study will contribute to the literature by using Triglyceride/Glucose index and traditional indicators of obesity together to screen patients for insulin resistance, since insulin measurement is limited in primary healthcare institutions. Method: The population of our study consists of people between the ages of 18-65 who applied to the Obesity Polyclinic of Tokat Gaziosmanpaşa University Faculty of Medicine Research and Application Hospital between September 2021 and September 2023. The examination and test information of the patients, from their first application to their application 6 months later, were retrospectively examined, and the study was created by taking the blood tests and measurements in their files through the institution's information system. Data from patients were analyzed using SPSS 27 for statistical computations. Finding : The average age of the 82 people in our participant group was 42. 74 (90.25%) of this participant group are women and 8 (9.75%) are men. 69 (84.15%) of our participant group are married and 13 (15.85%) are single. It was observed that 73 people (88%) in our participant group did not actively smoke and 80 people (97.60%) did not drink alcohol. Although there were significant decreases in waist circumference measurements at baseline and at the 6th month in both men and women, it was determined that there was no significant change with HOMA data in both applications (Waist Circumference (0th Month) p value 0.679 and Waist Circumference (6th Month) p value 0.787. ). In our study, it was determined that there was a positive correlation between the Triglyceride Glucose index and HOMA-IR data. It was observed that as the Tyg Glucose index increased, the HOMA score increased (r = 0.341, p = 0.002). Results: In line with the data obtained in our study, the relationship between obesity indexes and HOMA-IR data was examined and its usability in predicting first-line insulin resistance was evaluated. As a result, when examined, it can be seen that there may be a relationship between traditional indices of obesity and insulin resistance, in line with the data found in our study, but many studies are needed to determine strong correlations and to evaluate them as a routine in clinics.
