Preoperative transdermel fentanyl effect of postoperative analgesic consumption orthopedic hip prosthesis surgery

Abstract (EN)

Postoperative pain is an acute pain that begins with operative trauma, slowly decreases and ends with tissue healing. Treatment of this acute, distressal and anxietous situation should be rapid and effective. Furthermore being a responsibility, treatment of this situation is a humanitarian responsibility.Patients between 30-80 years old and ASA 1-3 who has had total hip arthroplasty for chronic coxarthrosis was choosen. For the study group, 12/24 ?cg/saat fentanyl (12 ?cg/saat 50-100 kg patients, 24 ?cg/saat >100kg patients) patch was given 12 hours before operation. All patients has had standart anesthesia induction, maintenance and postoperative analgesia. Premedication has been made 30 min before induction for all patients and sadation scores before induction and postoperative 0., 15. mins 1., 2., 4., 6., 12. and 24. hours was evaluated, adverse effects recorded. Preoperative and postoperative VAS scores and meperidine uses recorded. PCA use at postoperative 0., 15., 30. minsand 1., 2., 6., 12., 18., 24. hours.After evaluation of these findings, we see that, with the usage of 12 µgr/hour preemptive transdermal fentanyl, at 15., 30. mins and 6., 12. hours VAS score was reduced, meperidine need was reduced and 15. min sedation score was reduced too. But we see that preoperative sedation score and PCA usage was no different. We evaluated adverse effects, nausea was seen %16.7 at control group and %33.3 at study group but no significant difference was seen.Based on this study, usage of 12 µgr/hour transdermal fentanyl 12 hours before operation is easy and safe for patients who have chronic pain because of chronic coxartrosis and will have total hip arthroplasty.
