Özet (EN)

OBJECTIVES: Our study was preapared as a medical speaciality thesis which is about the relationship between serum leptin level; bone mineral density (BMD), body mass index, some chemical parameters and the risk of fracture in osteoporotic patients who have fractures on the proximal end of femur. Our aim is to provide contribution from our country for the investigations that are continue all over the world. Still it is not clear that the level of leptin is a marker for the fractures or not. İn this study we investigate the realtionship between leptin and bone mineral density and the risk of bone fracture in patients who have fractures on the proximal end of femur.MATERİAL AND METHODS: There were 25 patients (11 male,14 female) who have fractures on the proximal end of femur in the case group. The patients who have trauma stories, afected by bone cancer, secondary osteoporozis and get systhemic steroid treatment, medical treatment for osteoporosis haven?t been in this case group. In control group; there were 25 patients who (8 male,17 female) have been in our clinic because of several reasons but not the pathologyic fractures. All cases have DEXA and BMD measurements of lomber 1-4 spine and the 5 regions of proximal femur. We also had the levels of osteocalcin, PTH, 25(OH)vitamin D and leptin of each cases. The lenght and the weight of all cases had been recorded and their body mass indexes had been calculated. For the statistical analysis of the data, student t, khi-kare, Mann Whitney U and Pearson correlation tests had been used in SPSS 11.5.RESULT: The mean level of leptin was 32,68±31,9 ng/ml in control group but the level of leptin was 19,98±33,6 ng/ml in cases who had fractures. The mean level of leptin was 35,33±38,2 ng/ml (n=31) for women and 11,06±13,5 ng/ml (n=19) for men. There was a positive correlation between BMI and leptin for each cases (r=0,435, p=0,002). We found a positive correlation between leptin and the T score (r=0,299, p=0,35) of the thoracantheric part of femur. In control group there was a positive correlation between BMI and serum level of leptin (r=0,532, p=0,006) but not in case group.CONCLUSION: We found a positive correlation between BMI and serum level of leptin. The high leptin levels of the cases in control group support the hypothesis which is about the prevention of fractures by the high levels of leptin. The positive correlation between BMD and some anatomic regions shows that it is one of the positive factors for BMD. It is a discussion that the serum levels of leptin could be used as a predictor test to consider the risk of fracture.KEY WORDS: Leptin, BMI, BMD, osteoporosis, the risk of fracture.
