Evaluation of hemogram parameters in osteoporosis

Abstract (EN)

AIM: Osteoporosis is a metabolic bone disease characterized by decreased bone mass and deterioration in microarchitecture. The relationship between osteoporosis and inflammation is frequently emphasized. In our study, the difference between the neurophile/lymphocyte ratio(NLR),platelet/lymphocyte ratio(PLR),monocyte/ lymphocyte ratio (MLR) values, which were evaluated as inflammation parameters, between osteoporosis, osteopenia and normal groups; The correlation between osteoporosis and inflammation was tried to be shown by looking at the correlation between lumbar and femur T score values and NLR, MLR, PLR values. METHOD: 2019-2021 DEXA results and hemogram parameters, NLR, PLR, MLR values, erythrocyte sedimentation rate, C-reactive protein (CRP), vitamin D, calcium, phosphorus, parathormone (PTH), alkaline phosphatase (ALP), uric acid and B12 values were recorded. Patients with rheumatic diseases, malignancies, steroids or those using drugs that can change hemogram parameters were excluded from the study, and 250 patients were divided into 3 groups according to BMD measurement: osteoporosis, osteopenia and normal. The difference between the groups in NLR, PLR, and MLR and their correlation with the lumbar and femoral T score were examined. Distribution of variables in statistical analysis kolmogorov measured with the smirnov test. ANOVA, Kruskal-wallis and mann-whitney u test were used in the analysis of quantitative independent data, chi-square test was used in the analysis of qualitative independent data, and fischer test was used when the chi-square test conditions were not met . SPSS 28.0 program was used in the analysis. RESULTS: Of the patients, 101 were classified as osteoporosis, 100 with osteopenia and 49 as normal. While lymphocyte and monocyte levels were significantly lower in the osteoporosis group (p<0.05) than in the other groups, neutrophil percentage, NLR and PLR values were found to be significantly higher. NLR, PLR, MLR values did not correlate with sedimentation and CRP (p>0.05). Lumbar and femoral T score and PLR value showed a negative correlation. (r= - 0.250, p=0.00; r= -0.134, p= 0.035) CONCLUSION: In our study, NLR and PLR were found to be significantly higher in the osteoporosis group compared to the other groups, and the lumbar and femoral 6T score values were negatively correlated with PLR, suggesting that osteoporosis may be a disease associated with an inflammatory process, comorbid suggested that inflammatory diseases may deepen osteoporosis.
