The effect of the teflon piston diameter and piston stabilization with bone cement in otosclerosis surgery on the hearing results

Abstract (EN)

In this study, evaluation of hearing results and functional hearing success rates after otosclerosis surgery, evaluation of developed complications and approach to the complications, reasons and results of revision surgeries, comparison of the effects of teflon piston diameters used in the operation and bone cement application between the incus and piston on hearing results and functional success rates, to be discussed in the light of the literature was aimed. In this study, 64 patients (71 operated ears) who werediagnosed with clinical otosclerosis and operated due to conductive type hearing loss between September 2012 and August 2020 in Gaziantep University Department of Otorhinolaryngology were analised. Postoperative air bone gap was used to evaluate the functional success of the hearing results of the patients.Postoperative air-bone gap of 20 dB or less was considered as functional success criteria.The patients were divided into two groups according to the diameter of the teflon piston used in the operation, as 0.4 and 0.6 mm.At the same time, they were also classified according to the use of bone cement between the incus and the piston during the operation.During or postoperative complications were evaluated and recorded.Revision reasons and operation results of patients undergoing revision surgery were evaluated.Functional success was achieved in 54 (76%) of 71 operated ears. The success rates according to the piston diameters were 75% and 84.5% for 0.6 mm and 0.4 mm, respectively. The success rates according to the use of bone cement were 80% for the cement used group and 71.4% for the unused group. Revision surgery was performed in 6 patients (8.4%) and the success rate was 66.6%. Stapedotomy is a successful surgical method with low complication rate and is applied in the treatment of conductive type hearing loss in otosclerosis. As a result, no statistically significant effect of piston diameter and bone cement use on functional hearing success was observed.
