Abstract (EN)
Aim: Ductus arteriosus is an important anatomical structure for the baby to live in intrauterine period and it closes immediately after birth. Ductus arteriosusus remains open after birth is called patent ductus arteriosus. Patent ductus arteriosus can cause serious adverse conditions on the health of the baby. Severe patent ductus arteriosus can even cause death of the baby. For this reason, it is very important to close it early. Conservative methods, drug therapy, ligation therapy and surgical methods can be used for the treatment of patent ductus arteriosus. The most commonly used drugs are ibuprofen, indomethacin and paracetamol. Although most of the mechanisms that cause patent ductus arteriosus are known, some important factors are still unknown. Hemogram parameters are one of the cheapest laboratory parameters used by physicians. In this study, it was aimed to evaluate hemogram parameters in newborn patients with Patent ductus arteriosus. Materials and Methods: Cases were consist of the preterm babies with hemodynamically significant patent ductus arteriosusi have full blood examination results in their files and fallowed up in the Dicle University Faculty of Medicine Hospitals, Child Health and Diseases clinic, Neonatology clinic between January 1, 2014 and August 1, 2019. Ethics approval was obtained for this study from the ethics committee of Dicle University Faculty of Medicine. Findings: Patients with Patent ductus arteriosus included 93 cases as 48 (51.6%) girls and 45 (48.4%) boys, and 53 (53%) girls and 47 (53%) who were taken as control group. It consisted of a total of 100 cases, 47% of whom were male. When patients with patent ductus arteriosus are evaluated according to their general characteristics, 7.5% cesarean, 18.3% twin twin pregnancy, steroid in antenatal period 22.6%, PDA recurrence rate 7.5%, mortality 23.7% and the rate of patients who underwent surgical ligation 6.5%. was found. Oral ibuprofen 43.0%, IV ibubrofen 9.7%, oral paracetamol 10.8% and IV paracetamol 36.6% treatments were administered as patent ductus arteriosus closure drug. A statistically significant difference was found between hemoglobin, hematocrit, PDW, MPV, lymphocyte and neutrophil values at the time of diagnosis and after drug treatment of patients with patent ductus arteriosus (p≤0.05). A statistically significant difference was found between hemoglobin, hematocrit, MPV and WBC values at the time of diagnosis of cases with patent ductus arteriosus and cases without patent ductus arteriosus (p≤0.05). Conclusion: As a result, hemoglobin, hematocrit, PDW, MPV, lymphocyte and neutrophil values, which are among the most frequently used hemogram parameters, can give us an idea about following patients in which patent ductus arteriosus can be seen more frequently and which patients will respond better to single cure drug treatment. However, we believe that more comprehensive studies on this subject will be beneficial. Keywords: Newborn, patent ductus arteriosus, hemogram parameters.
