Comparison of videolaringoscopy and direct laringoscopy in pediatric elective surgery

Abstract (EN)

In this study, it is aimed to demonstrate whether videoryngoscopy is advantageous compared to direct laryngoscopy in terms of intubation time, anterior laryngeal compression, number of intubation attempts and hemodynamic changes in pediatric elective surgeries weighing 10-40 kg, which are not expected to have a difficult airway. 100 patients between 0-18 years who were planned to be operated under general anesthesia and having ASA I-II-III risk score were included in the study.Patients' age, gender, height, weight, BMI (body mass index), Mallampati score, ASA score, and type of surgery were recorded.The patients were divided into two groups by randomization method. 50 patients were intubated by video laryngoscopy and 50 patients by direct laryngoscopy. Heart rate, systolic blood pressure (SAB), diastolic blood pressure (DAB), mean blood pressure (MAP), SPO2 values were recorded before and after intubating the patients. Intubation time, anterior laryngeal compression, number of intubation attempts, intubation success at first attempt, intubation success, complications and Cormack Lehanne score were measured in patients. In tubation times of the patients in the videolaryngoscopy group were found to be statistically significantly higher whereas the need for anterior laryngeal compression and the intubation success rate at the first attempt were statistically significantly lower. In the intragroup evaluation, pulse, SAB, DAB and AOB were found to be statistically significantly higher after intubation than before intubation in both groups, but there was no significant difference in SPO2 value. In the evaluation between the groups, pulse change, DAB change and AOB change were statistically higher in the videolaryngoscopy group compared to the direct laryngoscopy group, but no significant difference was observed in the change in SAB and SPO2. In conclusion, while less anterior laryngeal compression is needed in videolaringoscopy in pediatric patients, more time and number of attempts are required for intubation. It has been shown that videoryngoscopy does not provide an advantage in pediatric cases with normal airway.
