Abstract (EN)
Introduction: Pylonidal sinus is an illness that occurs in the sacrococcygeal region of body. This illness is generally observed often in young males with high density of body hair. It can be asymptomatic, acute abscess or in chronic form.Treatment is determined based on the clinical situation of the illness. The purpose of our study is finding the risk factors in Pylonidal sinus and the relation between operation method, patient-dependent parameters and complications after operation. Tool and method: 127 cases, which were treated and operated at Yeditepe University General Surgery Department between 05/01/2007-08/01/2013, were studied retrospectively. The cases were categorized in two groups; Pilonidal sinus excision + Cases that were treated with V-Y plasty (Group 1), Pylonidal sinus excision + Limberg flep treatment (Group 2). We studied the dependence of observations for both groups on age, gender, height, weight, BMI, cigarette and alcohol consumption, duration of complaints, existence of other illnesses, existence if Pylonidal sinus illness in family, daily sitting period habits, weekly showering habits, duration of operation, duration of hospital stay, existence and duration of drain, duration of painless walk and sitting at the toilet; the effect of cigarette and alcohol consumption, existence of other illnesses, daily sitting period habits, weekly showering habits, existence and duration of drain, the material and technique used for covering the skin on complications. Observations: In both groups, cases occur for males substantially more frequently than females. A difference has not been observed between two groups with respect to age, height, BMI, cigarette and alcohol consumption, existence of additional illnesses, existence of Pylonidal sinus illness in patient's family, duration of hospital stay, existence and duration of drain, duration of hospital stay, duration to start to work, and existence of complications. The illness has been found to occur substantially more often in those who sit longer than 4 hours a day and who take a shower 3 times or less per week. In addition, patients in group 1 have been found to have shorter time to walk without pain and shorter time to sit at the toilet, with respect to those in group 2. Among 13 patients who had complications; 4 smoked cigarette, 2 drank alcohol, and 2 had additional illnesses. 11 of these patients had showers 4 or more times a week and 2 had 3 or less showers per week. 11 of these patients sit longer than 4 hours per day, 2 sit shorter than 4 hours per day. Among 13 patients with complications, 10 did not have drain, and 3 had have drain installed, 6 patients' skins were covered with prolen, 4 patients' skins were covered with rapid vicryl, and 3 with stapler. Conclusion: The observations in groups 1 and 2 did not differ with respect to gender, height, weight, BMI, cigarette and alcohol consumption, existence of Pylonidal sinus illness in the family, existence of additional illnesses, duration of complaint, duration of operation, duration of hospital stay, existence and duration of drain, time to starting work, existence of complications. The illness occurs more often for those patients who shower 3 times or less per week and inadequate personal hygiene plays and important role in the progression of the illness. The illness has been found to occur in those who sit longer than 4 hours a day and in those who have long sitting habits. Frequent showering and sitting not for long everyday can be methods of protection from Pylonidal sinus illness, although they don't not guarantee complete protection. Frequent showering habits after operation, however, have been found to increase the risk of developing complications after operation. Not having long sitting habits during the day has also been found to reduce the risk of complications. Group 1 patients have been found to sit shorter in the toilet without pain and walk shorter without pain, with respect to those in group 2. Consumption of alcohol and cigarette and existence of additional illnesses have also been found to increase the risk of the development of the illness.
