Abstract (EN)
In our study, the effect of iv magnesium sulphate treatment on the spinal anaesthesia produced by bupivakain, a local anaesthetic from amide group, in normal and preeclamptic patients were investigated.Following written informed consent from the Ministry of Health Ethical Committee, 64 pregnant (32 normal and 32 pregnant with preeclampsia) undergoing elective caesarean section under spinal anaesthesia were accepted for study. Normal and preeclampctic pregnants were divided into four groups as patients given iv magnesium sulphate treatment and as control. Group I (n=16) normal patients were given 5 % dextrose infusion, group II (n=16) normal patients were given iv magnesium sulphate infusion, group III (n=16) preeclamptic patients were given 5 % dextrose infusion and group IV (n=16) preeclamptic patients were given iv magnesium sulphate infusion. Spinal anaesthesia was induced with 12.5 mg 5 % hyperbaric bupivacain from the T3-4 interspinöz space. Intraoperative and postoperative hemodynamic variables, sensorial block periods, onset times of sensorial and motor block, maximum sensorial block levels, the time to teach maximum block level, bromage scores, consumptions of intraoperative analgesic and ephedrine, the quality of anaesthesia, the duration of spinal anaesthesia and magnesium levels of blood and serebrospinal fluid (CSF) were measured and recorded.The level of magnesium in blood and CSF were found significantly higher in group given magnesium compared the groups who did not give magnesium in preeclamptic patients (p<0.01). Systolic and diastolic artery pressures were found significantly higher in group III and IV at the preoperative and intraoperative 1st, 5th, 15th, and 30th minutes than in group I and II (p<0.05). Onset of sensory and motor block times were significantly longer and the duration of anaesthesia was shorter in group II and IV given magnesium treatment than in group I and III (p<0.05). Although the quality of anaesthesia was similar, supplement analgesic consumption was significantly higher in preeclamptic pregnants given magnesium sulphate than in preeclamptic pregnants who did not given magnesium sulphate (p<0.05).In conclusion; the treatment of iv magnesium sulphate during the spinal anaesthesia produced by bupivacain exteded onset of sensory and motor block, shortened duration of spinal anaesthesia and therefore, provided early analgesic requirement.Key words: Spinal anaesthesia, magnesium sulphate, preeclampsia.
