Determination of factors affecting QT dispersion in patients with ST segment elevated myocardial infarction

Acute ST-segment elevated myocardial infarction may proceed with life-threatening ventricular arrhythmias. QT dispersion is a well known pathogenetic factor for ventricular arrhythmias representing heterogeneity of repolarization. In our study, we aimed to determine factors affecting QT dispersion, the impact of primary percutaneous intervention on QT dispersion and the course of QT dispersion in-hospital period in patients with ST-segment elevated myocardial infarction. Patients with no prior coronary artery disease, diagnosed in the emergency department who underwent successful primary percutaneous intervention between May 2011-May 2015 were reviewed retrospectively. Enrolled 171 patients had an average 81.1 ± 0.81 ms QTc dispersion, respectively. The average QTc dispersion declined to 70.2 ms after successful revascularization. At the time of diagnosis QTc dispersion of anterior myocardial infarction group was 84.8 ± 1.19 ms, while inferior myocardial infarction group was 78.3 ± 1.05 ms. (p <0.001) This difference was continued in-hospital period between two groups. In terms of severity of coronary artery disease patients with high gensini score had higher QTc dispersion than patients with low/medium score. (84.6 ms - 77.9 ms p <0.001) Patients with multi-vessel disease also had higher QT dispersion than patients with single-vessel disease. (p <.01) Patients with left ventricular ejection fraction below 50% had higher QTc dispersion compared with patient who have normal ejection fraction. (p <.01) Patients who developed pathological Q waves also had higher QT dispersion compared with patients without a Q wave. (p <.01) There is no statistically significant affect of hypertension or diabetes on QT dispersion in patients with acute myocardial infarction (p> .05) As a result, increased QT dispersion in acute myocardial infarction decreases significantly with successful primary percutaneous intervention.
