The relationship between pulmonary arterial obstruction index ratio and monocyte/HDL ratio in pulmonary thromboembolia patients and the effect of these parameters on SHORT-TERM prognosis

Abstract (EN)

Introduction and Aim: Pulmonary tromboembolism (PTE) is a difficult disease to diagnose, so acute PTE cannot be diagnosed on the basis of clinical symptoms and findings alone. In our study, pulmonary arterial obstruction index (PAOI) measurement was performed radiologically to determine the severity of the disease in patients diagnosed with pulmonary tromboembolism (PTE) by computed tomography pulmonary angiography (CTPA). Within the scope of the study, it was aimed to evaluate the relationship between radiological PAOI ratio (PAOIR) measurement and monocyte/HDL ratio in patients and to determine the effect of these parameters in predicting short-term (30-day) mortality. In addition, it was aimed to evaluate the contribution of other biomarkers, primarily the sPESI value, to the determination of the clinical severity of the disease. Material and Method: Within the scope of the research, clinical and laboratory findings of 356 patients aged 18-100 years who were diagnosed with pulmonary tromboembolism by computed tomographic pulmonary angiography in Erzincan Binali Yıldırım University Faculty of Medicine Mengücek Gazi Training and Research Hospital Chest Diseases Service were examined. Results: The mean age of the participants in the study was 68.39±16.45 (24-98) years. In the study, the sPESI index value was 2.38±1.46; PAOIR index value was 38.21±17.58; The PASB value was found to be 42.21±15.94, and the MHR as 20.25±14.04 to be moderate. 13.2% of the patients died within a month. It was determined that the laboratory and clinical findings of those who died in mortality status were higher than those who survived. In the correlation analysis, there was a moderate positive correlation at 99% confidence level between the PAOIR value and PASB and sPEI (r=0.575, r=0.514); A very weak positive correlation was found between the PAOIR value and the monocyte/HDL ratio at 99% confidence level (r=0.142). In the ROC analysis for monocyte/HDL ratio and PAOIR value, the AUC values of the variables were around 0.6. These values showed that the PAOIR and MHR parameters had weak capacity to distinguish between positive class and negative class. Conclusion: There is a weak positive correlation between PAOIR value and MHR. The effect of these parameters in determining the prognosis is weak, and it is thought that more longitudinal studies and testing the relationships with other parameters are needed.
