The value of serum ?ıschaemıa-modıfıed albumın? (IMA) for dıagnosıs and relatıon to prognosıs ın pulmonary thromboembolısm

Aim: In our study, we aimed to find out the diagnostic value, the relationship between the severity of PTE or clinical and radiological findings and IMA and IMA/albumin ratio in pulmonary thromboembolism (PTE) patients.Method: 99 volunteer patients who applied to Dokuz Eylul University School of Medicine, Pulmonary Diseases Department or who were examined by our clinic, investigated for PTE included in the study. In the first 24-hours, serum from blood sample was separated and saved to analyze IMA and albumin. Patients? demographic datae, laboratory and radiological findings, clinical prediction scores were noted on patient recording forms. Patients were followed up at one-year, for the prognosis.Results: In our study, PTE group, in which the thrombi detected at CT pulmonary angiography, is 43-patients and control group which thrombi is not detected is 53 patients. The mean plasma IMA levels were 0.93±0.31 in control group, 1.05±0.35 in PTE group. (IMA; p value: 003) For the IMA/albumin, mean plasma levels were 0.27±0.10 in control group and 0.31±0.09 in PTE group. (IMA/albumin; p value: 0.01). According to PTE severity; nonmassive PTE 32 patients (74%), submassive 7 patients (16%) and massive 4 patients (9%) were defined. Massive PTE groups have higher IMA anda IMA/albumin results than submassive and nonmassive groups. (IMA; in nonmassive group 1.02±0.3, in submassive group 1.04±0.2, in massive group 1.33±0.4, IMA/albumin; in nonmassive group 0,30±0,07, in submassive group 0,291±0,04 and massive group 0.426±0.16). PTE group has 33% mortality and 7% recurrence ratios. We observed IMA and IMA/albumin measurements are not significant for the prognostic value.Conclusion: IMA and IMA/albumin, as a new biyomarker, may be used in diagnosis and severity of PTE. Rather than alone use of this marker, like D-Dimer, such parameters which are accepted useful, can be included in algorithms and the use of this marker in these algorithms or as a part of combined test may be more helpful for diagnostic value.
