Perinatal outcomes in women with recurrent spontaneous abortus history

Abstract (EN)

Aim: In this study; it was aimed to investigate the sociodemographic and some clinical characteristics of pregnant women with and without a history of recurrent spontaneous abortion (RSA), and to reveal the perinatal outcomes in their subsequent pregnancies. Material and Methods: In this retrospective study; The data of pregnant women between the ages of 18-45 who gave birth for 20 weeks or more, and applied to the Gynecology and Obstetrics Clinic of Akdeniz University Medical Faculty Hospital between June 2016 and June 2020, were accessed from the hospital automation system and patient files. Patients were divided into two groups, with or without a history of RSA, and compared in terms of demographic characteristics, some hematological and biochemical parameters. In addition, the characteristics of the newborn (weight, height, FBK, cord blood gas pH value, 5th minute Apgar score), the need for a neonatal intensive care unit (NICU) and the gestational week at birth were also recorded and evaluated. It was applied to the logistic regression model to investigate the risk factors affecting the RSA risk levels. Results: The mean age of the pregnant women examined within the scope of the study was 30.31±5.54 years, and the gestational week at delivery was 38.06±2.07. While the gestational age and number of pregnancies in the patient group (n=210) were significantly higher than the control group (n=163), it was found that the mean of the gestational week at delivery, the number of births and the number of living babies were significantly lower (p <0.05). The APGAR and pH scores and hematocrit, platelet and PT values were found to be statistically significantly lower in the patient group compared to the control group (p <0.05). The rates of diabetes (22,4%), hypertension (15,7%) and intrauterine growth restriction (FBK) (16,2%) during pregnancy in the patient group compared to the control group (2,5%, 0,0% and 6,1%, respectively) were found to be statistically significantly higher (p <0.05). According to the logistic regression model applied; diabetes during pregnancy, hypertension during pregnancy, FBK, normal delivery, gestational age, gestational week were found as independent risk factors. Conclusion: The present results show that any pregnancy occurring after the first abortion is associated with a somewhat increased obstetric and perinatal risk. Preventable and controllable problems such as diabetes and hypertension were found to be more common in patients with a history of recurrent spontaneous abortion. For this reason, the detection, close follow-up and treatment of such chronic diseases should be provided.
