Levels of thiol disulfide in patients with rheumatoid arthritis and its association with oxidative stress and clinical parameters

Rheumatoid arthritis (RA); etiology is unclear, chronic, inflammatory and multisystemic. An increase in pro-inflammatory factors in the reactive oxygen system results in an imbalance in oxidative homeostasis, which plays a strong role in the formation of oxidative stress. Thiols, an important antioxidant, enter oxidation reactions with oxidant molecules and form disulfide bonds. Dynamic thiol / disulfide homeostasis is necessary for detoxification, regulation of signaling pathways, apoptosis and regulation of enzymatic reactions. In this study, we aimed to evaluate the relation of thiol / disulfide levels with oxidative stress and clinical parameters in patients with rheumatoid arthritis. The study included 93 patients and 30 healthy volunteers who met the 2010 ACR / EULAR Rheumatoid Arthritis Classification Criteria. In the evaluation of patients; WBC (White Blood Cell Count), sedimentation, CRP (C-Reactive Protein), HAQ (Stanford Health Assessment Questionnaire) and SF-36 (Short Form-36) forms were used. Serum NT (Nativ Thiol), TT (Total Thiol), TAS (Total Antioxidant Stress) and TOS (Total Oxidant Stress) levels were measured by enzyme-linked immunosorbent assay (ELISA). Disulfide (D); It was calculated as half of the difference between TT (Total Thiol) and NT (Nativ Thiol). OSI (Oxidative Stress Index) was calculated as TOS / TAS. NT / TT, D / NT and D / TT ratios were calculated. In RA patients, NT, TT, disulfide and TAS values were significantly lower compared to controls (p <0.05). OSI ratio was close to the significance level in patients with RA compared to healthy controls (p:0.05). There was a significant inverse correlation between DAS-28 disease activity score with NT and DAS-28 disease activity score with TT level (p <0.05). There was no correlation between NT, TT, disulfide levels, NT / TT, D / NT, D / TT ratios and TAS, TOS, OSI and SF-36 Total values of the patients. There was a significant inverse correlation between NT, NT / TT, TT and sedimentation. There was a significant positive correlation between D / NT, D / TT and sedimentation (p <0.05). Significant positive correlation between HAQ scores and D / TT, D / NT ratios of RA patients. There was a significant inverse correlation between HAQ scores and NT / TT ratio. As a result; we investigated the relationship between Thiol / Disulfide balance, oxidative stress and clinical parameters in rheumatoid arthritis. Sedimentation, CRP, WBC, NT, TT, Disulfide, TAS and OSI values in the patient and control group we found statistically significant difference. There was a correlation between some clinical and laboratory parameters in Thiol / Disulfide balance. There is a need for extensive participation studies in order to better understand and use these parameters in pathogenesis, rheumatoid arthritis, clinical course and follow-up.
