Factors affecting recurrence in patients operated with the diagnosis of sacrocoxygeal pilonidal sinus

Abstract (EN)

Aim: Pilonidal sinus disease is common in our society; asymptomatic, which can have acute chronic forms; It is a disease that affects social life. Unfortunately, an effective and valid treatment protocol has not been adopted today. The aim of our study is to examine the etiological factors and surgical techniques in patients treated for pilonidal sinus disease in our clinic; It is to determine effective treatment with low recurrence rate. Materials and Methods: Our study retrospectively analyzed 297 patients diagnosed with pilonidal sinus and admitted to Erzincan Binali Yıldırım University (EBYU) Faculty of Medicine, Department of General Surgery between January 2013 and November 2020 for treatment. These patients were asked questions about various etiological factors (complaint, duration of complaint, history, family history, abscess history, BMI…). Afterwards, 75 of the operated patients were reached. It was learned that 6 of these 75 patients were re-operated for recurrent pilonidal sinus. Since these data are binary variables (recurrence, no recurrence), Binary Logistic Regression analysis; With this analysis, it was calculated how many times it increased or decreased the recurrence rate in the dependent variable. Chi-square analysis, correlation analysis, and optimal scaling method were used for some evaluations. Results: When the relationship between recurrent pilonidal sinus disease and etiological factors is examined; relapse was not associated with smoking, abscess history, number of weekly baths; those who were male, treated with treatment options other than reverse flap, those with high BMI, and those with multiple sinuses had more recurrences; We compared the specimen volume of those with a pit closer than 5 cm to the anal canal, with a complaint duration of less than 1 month more than 1 month, as two different values; It was determined that those who had more than 5 cc more than 20 cc and those who shaved anus had less recurrence. Conclusion: In our study, the relationship between etiological factors and recurrence was evaluated. As a result of this evaluation, in pilonidal sinus disease; We found that etiological factors such as long duration of complaints, personal hygiene, obesity, complicated pilonidal sinus cause more recurrences.
