Effect of modified NUTRIC score on prognosis in patients with severe SARS-Cov-2 in intensive care unit

Abstract (EN)

COVID-19 is a viral infection that mainly attacks the respiratory tract.Patients admitted to intensive care for COVID-19 are at higher risk of malnutrition, and nutritional status is an important factor in the course of illness for COVID-19 patients. Treatment of critically ill COVID19 patients is a long process and nutritional support is an important part of treatment. According to epidemiological data, malnutrition causes mortality and morbidity by greatly increasing the risk and severity of infection.The Nutritional Risk in the Critically Patient (NUTRIC) score was developed by Canadian researchers for intensive care patients. The American Society of Parenteral and Entral Nutrition (ASPEN) has recommended the use of the NUTRIC score in patients under intensive care followup.This scoring system includes age, Acute Physiology, Age, and Chronic Health Evaluation score (APACHE II score), The Sequential Organ Failure Assessment score (SOFA score), number of comorbidities, length of hospital stay before ICU, and serum interleukin-6 (IL- 6) levels. In another version of the NUTRIC score, since the serum IL6 level was not measured in many centers like ours, it was excluded from the scoring and named as the modified NUTRIC (m-NUTRIC) score. Our study was carried out retrospectively and in a single center in order to investigate the strains diagnosed with SARS-CoV-2 and the prognostic environments of the modified NUTRIC score. A total of 113 patients, 69 men and 44 women, were included in the study. According to the statistical data of 113 patients we evaluated in terms of modified NUTRIC score; gender, presence of diabetes, presence of hypertension, presence of cough symptoms, use of antihypertensive medication, exitus, age, blood creatinine level, need for dialysis, need for vasopressors, Glasgow coma scale score data APACHE II score, SOFA score, DIC score, diastolic blood pressure, A statistically significant difference was observed between patients with a modified NUTRIC score above and below 5 according to blood glucose level, BUN level, urea level, blood pH level, blood neutrophil count, blood monocyte count, blood basophil count parameters, and this supports the relationship between immunity and nutrition. Especially in immunosuppressive individuals who are at high risk for viral diseases, making nutritional recommendations as primary prevention and recognizing nutritional deficiency at an early stage should be integrated into our clinical practice, as it will reduce health expenditures and population-based mortality and morbidity.
