Abstract (EN)

Changes in cardiovascular and pulmonary system due to obesity in pregnants, may cause the obstetric and nenatal risks increase. Decrease in functional residual capasity below to the closing capasity can cause hypox-emia more frequently seen in arterial blood gas via atelectasia formation at the lower parts of lungs. These changes affect the oxygen content of mater-nal uterine artery; oxygen going to fetus, depends on the function of placenta, maternal arterial oxygen content and uterine blood flow. In our study, we search for the effect of perioperative noninvasive me-chanical ventilation to maternal and neonatal health in obese pregnant women who underwent cesearian operation. 69 subjects whose BMI ≥ 30 and BMI < 40 and who electively under-went cesearian in Celal Bayar University Hafsa Sultan Hospital operatian rooms were included into the study after obtaning the approval of the scien-tific ethical committee 04.06.2014/230. 9 subjects who couldn't tolerate the NIMV were excluded from the study. After taking consent, the subjects were divided into two groups. In group 1 (n=30), face mask and non-invasive me-chanical ventilation, in group 2 (n=30) 5 L/m O2 via mask was applied. Arteri-al blood gas samples from groups were taken both before and after the ap-plication. Umbilical arterial blood gas samples were taken afterwards the de-livery. APGAR scores of neonate at 1 and 5 minutes after birth were record-ed. At the same time, peripheral saturation of neonate was measured. All results were recorded to patient's history. In statistical examinations, student t test was used for comparing the study and control groups of continuous variables and t test in dependent groups was used for comparison of both study and control groups within each group. There is no significant difference between the mean ages, body mass indexes and gestation weeks of subjects. While there is no statistical differ-ence in preoperative arterial blood gas samples of subjects before applica-tion, after the application significant increase in pH values is observed. (p=0.047) Significant decrease is seen in HCO3 values (p=0.043). The anion gap values of umbilical artery blood in non-invasive mechanical ventilation applied group is detected as significantly low compared to the other group (p=0.007). There is no statistical significant difference between two groups according to APGAR scores (p>0.05). As a result, it's concluded that; applicaiton of noninvasive mechanical ventilation in obese pregnant women has positive effects on maternal health but doesn't have significant effect on neonates.
