Relation of ghrelin and cytokines levels in course of systemic inflammatory response syndrome advancing to severe sepsis.

Highly sensitive and specific laboratory tests are needed to guide clinicians in patients groups with the critical diseases such systemic inflammatory response syndrome and sepsis in which early diagnosis is very important, and according to diagnosis these situations require close monitoring response to treatment and changing treatment models when needed.In this study, we aimed to find out the marker which may define the course of systemic inflammatory response syndrome advancing to severe sepsis and to reveal the relationship between ghrelin having newly discovered functions in humans and inflammatory process.In our study, levels of glucose, CRP, lactate, deacyl and acyl ghrelin, TNF-?, IL-1ß, IL-6 and IL -10 were evaluated in the blood taken from 20 patients with similar demographic characteristics and with SIRS diagnosed according to the criteria of ACCP/SCCM consensus conference, 20 patients with severe sepsis and 20 healthy people in Department of Anaesthesiology and Intensive Care Unit, Firat University School of Medicine.As a result, we found that cytokines levels such as TNF-?, IL-1ß, IL-6, and IL-10 increased, but unlike other cytokines, especially IL-6 levels was found significantly increased in patients with SIRS and severe sepsis, and we considered that IL-6 level may be related to poor prognosis of the disease. Also we found that total ghrelin levels was lower in patients with severe sepsis when compared to SIRS. We concluded that IL-6 and ghrelin levels may be used as a predictor for the steps of inflammatory course in sepsis and related clinical tables but futher studies are needed in this matter.Keywords: Inflammation, severe sepsis, cytokines, ghrelin
